Submit A Device Protection+ Claim

How to file a claim if your phone or tablet is enrolled in Device Protection+ and the device is lost, stolen, broken or has suffered accidental damage, such as a cracked screen
Be Prepared With These Time-Saving Tips

Good to know:
  • For lost or stolen devices, call Customer Care at 1-888-944-9440 immediately to suspend your service and protect against unauthorized use
  • To file a claim, visit Submit a Claim.
  • For Apple customers enrolled in Device Protection+ Advanced with AppleCare service, visit for repair options
  • You may need additional documentation — such as a police report or proof of loss — to submit your claim
Make sure to have:
  1. The cause and date of device loss or damage
  2. Device model number and storage size
  3. Form of payment
    • You can check Submit a Claim to confirm deductible pricing or service fee for your device
  4. Any additional documentation necessary to process your claim, such as police report
